DVT / CBCT: What three-dimensional X-ray diagnostics can achieve — and when it is clinically necessary.

Q: Is a DVT necessary before every implant?

Not necessarily. In simple anatomical situations, a two-dimensional X-ray image may be sufficient. In more complex situations — proximity to nerve structures, reduced bone supply, multiple implants — DVT is the evidence-based standard. The decision is made by the treating dentist based on the individual situation.

Q: How high is the radiation exposure?

The radiation exposure typically ranges between 30 and 300 microsieverts depending on the device and image section (field of view). For comparison: a panoramic X-ray (OPG) is approximately 10–25 microsieverts, and natural annual background radiation in Austria is approximately 2,000–3,000 microsieverts. DVT is significantly lower in radiation dose than a medical CT scan.

Q: Will I receive my DVT data as a patient?

Yes. As a patient, you have the right to your diagnostic data — including DVT images in DICOM format. These can be provided on a data carrier and are important for a second opinion or further treatment. Ask for them proactively.

Q: Can I bring a DVT from another practice?

Usually yes — as long as the image is current, has sufficient image quality, and completely captures the relevant anatomical area. This avoids double irradiation and saves costs. Clarify in advance with the new treating dentist whether the DVT you bring is usable.

Digital Volume Tomography — DVT for short, internationally also known as CBCT (Cone Beam Computed Tomography) — is the most important imaging modality in modern implantology and complex dentistry. It provides three-dimensional images of bone, teeth, nerve pathways, and anatomical structures that cannot be displayed with conventional two-dimensional X-ray images.

How does DVT work technically?

The CBCT device rotates once around the patient's head, emitting cone-shaped X-radiation. A flat-panel detector captures hundreds of individual projections from different angles. Specialized software reconstructs a complete three-dimensional data volume — the DICOM volume — that can be viewed, measured, and fused with other datasets in any cross-sectional plane.

What does DVT show — and what doesn't it?

CBCT shows: bone height & width (implant planning), bone quality (implant stability), inferior alveolar nerve (safety distance), maxillary sinus (upper jaw planning), root canal morphology (endodontics), cysts & cavities, tooth germs & impactions, temporomandibular joint (TMD). It does NOT show: soft tissue, early caries, inflammatory soft tissue processes.

When is a DVT clinically indicated?

Implant planning with limited bone supply or proximity to nerve/sinus, planning of bone augmentation, complex endodontic situations, surgical removal of impacted teeth, assessment of cysts and pathological changes, temporomandibular joint diagnostics (TMD), complex orthodontic planning.

DVT and data fusion — the added value for implant planning

For complete digital implant planning, CBCT data (bone, nerves, anatomical boundaries) is digitally overlaid with the intraoral scan (tooth surfaces, soft tissue). This data fusion allows the implant position to be assessed in relation to all relevant anatomical structures — before the first surgical step is taken.
