In the following cases, implantation is generally not possible or requires very careful individual assessment by an experienced specialist:
Uncontrolled diabetes mellitus
HbA1c > 8% — significantly increased infection and wound healing risk
IV bisphosphonate therapy
Zoledronate, pamidronate — risk of jaw necrosis (MRONJ)
Jaw radiation therapy < 5 years
Irradiated bone tissue heals unreliably
Severe immunosuppression
E.g., after organ transplantation — infection risk not controllable
Active periodontitis
Must be completely treated before implantation
