"Does an implant last a lifetime?" — That is the question almost every patient asks before deciding on an implant. The honest answer: it depends. But the evidence is considerably more positive than most people expect.

What do the studies actually show?

Dental implants are among the most thoroughly researched dental treatments available. Long-term studies spanning 10, 15 and even 20 years exist — and the results are remarkably consistent.

Most large systematic reviews report a 10-year survival rate of 90–95 %. This means: of 100 implants placed, 90 to 95 are still functional after ten years. After 20 years, figures range between 80 and 90 % depending on the study.

What influences longevity?

Survival rates are not predetermined — they are influenced by several factors, a significant portion of which you as a patient can control yourself.

Oral hygiene

The single most important factor. Brushing twice daily, using interdental brushes and attending regular check-ups produces significantly better long-term results.

Smoking

Smokers have a 2–3 times higher implant failure risk. Quitting measurably improves the prognosis — even after the implant has been placed.

Diabetes

Well-controlled diabetes (HbA1c < 7 %) has little effect on survival rates. Poorly controlled diabetes increases the risk considerably.

Bone quality

Good bone means better primary stability and faster osseointegration. Bone augmentation before implantation improves the starting position.

Implant position

Lower jaw implants statistically last slightly longer than upper jaw implants due to differences in bone density.

Aftercare

Regular professional cleaning (every 6–12 months) substantially reduces the risk of peri-implantitis.

Survival vs. success — an important distinction

Science distinguishes between implant survival (the implant is still present) and implant success (the implant is present, functional, free of inflammation and without bone loss).

Success rates are slightly lower than survival rates — because an implant can still be in the jaw while simultaneously having peri-implantitis. This is not failure in the classic sense, but it does require treatment.

What is peri-implantitis?

Peri-implantitis is an inflammation of the tissue surrounding an implant — comparable to periodontitis around a natural tooth. It is usually caused by bacteria and, if left untreated, can lead to bone loss and ultimately implant failure. What happens when an implant fails?

Implants are the only tooth replacement option that most closely replicates a natural tooth in function, aesthetics and bone preservation. Bridges require grinding down healthy adjacent teeth. Removable dentures do not load the bone and lead to long-term bone resorption.

If you are healthy, do not smoke, maintain good oral hygiene and are willing to attend aftercare appointments, an implant is statistically the most durable solution available — with a realistic expectation of 15–25 years and more with good care.
