Why bacteria in periodontal pockets are the core problem
Periodontitis is a bacterial disease. Specific anaerobic bacterial strains — in particular Porphyromonas gingivalis, Treponema denticola and Tannerella forsythia — form biofilms in deep periodontal pockets that are difficult to reach by mechanical means alone. These bacteria produce enzymes and toxins that attack the gum tissue and bone.
This is precisely where the diode laser comes in: its wavelength (810–980 nm) is absorbed by the dark pigments of these bacteria and destroys them thermally — without damaging the surrounding healthy tissue. This is the principle of selective photothermolysis.
In a digitally equipped practice, laser treatment is integrated into a structured workflow: digital clinical documentation before treatment, laser application, and digital assessment of treatment outcomes.

Selective Bacterial Elimination
The diode laser precisely destroys bacteria within the periodontal pocket without damaging the surrounding healthy tissue. The wavelength (810–980 nm) is absorbed by bacterial pigments, not by healthy tissue.
No Antibiotic Required
In many cases, laser treatment allows systemic antibiotics to be avoided. This reduces the risk of antibiotic resistance — an increasingly important issue in dentistry.
Biostimulation of Healing
In addition to its antimicrobial effect, the laser stimulates cell division in the gum tissue. This promotes wound healing and can accelerate tissue regeneration following treatment.
Suitable for Use Around Implants
The diode laser is also suitable for treating peri-implantitis, as it does not damage titanium surfaces. This is an important advantage over mechanical instruments when caring for implants.
Typical procedure for diode laser treatment
Digital Clinical Assessment
Periodontal status including pocket depths, bleeding points and radiographic findings are recorded digitally. This baseline serves as a reference for evaluating treatment success.
Mechanical Cleaning (Scaling & Root Planing)
Calculus and hard deposits are removed mechanically. This forms the foundation of every periodontal therapy — the laser complements but does not replace this step.
Laser Application into the Periodontal Pockets
A thin laser fibre is introduced into the periodontal pocket. The laser light eliminates the remaining bacteria while simultaneously stimulating tissue healing. Depending on the number of pockets, the procedure takes 20–60 minutes.
Digital Outcome Assessment
At the follow-up appointment (typically after 4–6 weeks), pocket depths are measured digitally again. A comparison with the baseline findings objectively shows whether treatment was successful.
