Who needs a TMD diagnosis?

TMD symptoms are varied and are often not immediately associated with the jaw joint. The following warning signs should be evaluated:

Jaw Clicking & Pain

Clicking, grinding, or pain when opening and closing the mouth.

Restricted Mouth Opening

Mouth opening under 40 mm or deviation of the lower jaw midline when opening.

Chronic Headaches

Recurring headaches, temple pain, or ear pain without a clear cause.

Teeth Grinding (Bruxism)

Nocturnal grinding, worn tooth surfaces, or tension in the chewing muscles.

Diagnostic steps in detail

A comprehensive TMD diagnosis follows a structured process — from basic examination through to instrumental analysis.

Step 1

Medical History & Pain Record

Detailed consultation on pain duration, character, and triggers. Assessment of stress factors, sleep quality, and dental care habits. This forms the foundation of every TMD diagnostic process.

Step 2

Clinical Functional Analysis

Measurement of maximum mouth opening (normal range: 40–55 mm), bilateral palpation of the jaw joints and chewing muscles (masseter, temporalis). Assessment of occlusion and jaw guidance.

Step 3

Instrumental Condylography

Electronic recording of three-dimensional jaw joint movements. Enables precise analysis of the condylar path, range of motion, and asymmetries — the basis for splint therapy planning.

Step 4

Occlusal Analysis

Digital or manual analysis of tooth contacts in various jaw positions. Identification of faulty contacts (interferences) that may contribute to TMD.

Imaging & Differential Diagnosis

Depending on findings, imaging procedures are used and other conditions are ruled out.

CBCT / 3D X-ray

Bone

Cone beam computed tomography reveals bony changes to the condyle, joint space narrowing, and signs of arthrosis. Low radiation and high resolution.

MRI of the Jaw Joint

Soft Tissue

Gold standard for soft tissue diagnostics: shows disc displacement (with/without reduction), joint effusion, and inflammatory changes — without radiation.

Trigeminal Neuralgia

Exclusion

Differential diagnosis: brief, electric shock-like pain attacks without jaw joint findings. Neurological evaluation (cranial MRI) required. No TMD findings present.

Myofascial Pain

Differentiation

Overlap with TMD is possible: trigger points in the chewing and neck muscles. Palpation findings and pain radiation assist in differentiation.

Diagnostic Report: Once diagnostics are complete, you will receive a written report including diagnosis, severity classification (according to RDC/TMD criteria), and treatment recommendations.
